I understand there may be risks involved in volunteer participation in any activity and I voluntarily assume all such risks. I intend to be legally bound, do hereby, for myself, the student, heirs, executors, and administrators waive, release, and forever discharge any and all rights and claims for damages which may have or which may hereafter accrue to the student or myself against United Way of Southwest Virginia, the owners of facilities in which the events are held, or any other support group of organizations, and their respective directors, officers, agents, members, sponsors, parents, volunteers, representatives, successors, and assignees for all damages which are sustained and suffered by the student or myself in connection with his or her entry or participation in any event involving United Way of Southwest Virginia or any governing body participating in the event. I hereby grant permission to United Way of Southwest Virginia, and its employees or agents, to use and reproduce my image (either in video or photograph) and my communication (either verbal or written) without compensation to me to promote a United Way of Southwest Virginia service, initiative, idea, event, or program I understand that the materials may be used in a publication, print ad, direct-mail piece, electronic media (e.g., video, slide show, website), or other form of promotion or publicity effort. I release United Way of Southwest Virginia, the photographer, their offices, employees, agents, board, and designees from liability for any violation of any personal or proprietary right I may have in connection with such use.
